Evaluation of Emergency Room Triage Performed by Nurses

Abstract
This study experimented with the applicability of trige system performed by nurses at one university hospital for 3 months. The study team had developed an assessment tool and it was revised by expert and the results of pilot test. Triage nurses were selected and trained out of nursing staffs who had certification of paramedics and had experienced for two or more years in emergency room. The results are as follows.
1) In triage area, visiting patients were assessed and distributed by nurses within 9 minutes. The doctor in charge assessed patients initially at 34 minutes after the visit, and carried out lab-test and X-ray each at 24.6 minutes, 31.6 minutes After the experiment period, patients were assessed by doctor at 53.3 minutes and were tested laboratory and X-ray examination at 47.6 minutes, 70.2 minutes after the visit in the emergency room. It means that triage system is one of the solution to the stagnation in the emergency room.
2) The outcome predictability of triage classification by nurse was 98.3% in non-emergent case but 8.3% in urgent and emergent case. If the validity of classification evaluated in comparison with APACHE ¥± score as gold standard, sensitivity 63.3%.
3) Triage Nurses complained that the protocol of triage was not sufficient for performance, and were anxious about using the triage classification.
4) The statistically significant indexes of triage classification for the patient with chronic illness were status of consciousness, existing disease or not, severity of pain, hemorrhage or not and vital signs(diastolic pressure, purse rate, body temperature).
5) In order to settle a triage system as the university hospital, the training program and protocols have to be designed considering the philosophy, purpose and resources of the hospital.
